Request a Repeat Prescription

This facility is for REPEAT Prescriptions ONLY and cannot be used for once off medication requests.  If you require a prescription for a once off condition you must contact the Health Centre on (066) 9157227 regarding same. 

• All repeat prescription requests must be received in writing.
• The fastest way to submit your repeat prescription is via our website forms –  for patients with a valid Medical Card >>  or for private patients >> (for patients without a Medical Card).
• Your repeat prescription will be sent directly to your chosen pharmacy for collection.
• If you are having difficulty with the online form you can ask your pharmacy to email a repeat prescription request too , or you can return a completed form to the post box at Annascaul Health Centre, Annascaul , Co Kerry
• To avoid an interruption in your supply of medication, please submit your request at least 7 days before your next prescription is due.
• Please note that online Prescription Request facility is monitored in conjunction with our opening days/hours
